Getting Started

  • When your rebate poster arrives, write in the 6-digit alpha-numeric code, which is unique to your practice and received through email. It will look like this:

How it Works

  • DISPLAY THE POSTER

    Once you have written in your unique 6-digit code, simply place your Store Brand Formula rebate poster in the waiting area, or checkout space – whatever makes sense for your practice.

  • FAMILIES SCAN + REGISTER

    Parents scan the QR code, which takes them to a program website where they enter your office’s unique 6-digit code to register.

  • FAMILIES SHOP + SUBMIT

    Parents or caregivers purchase any qualifying Store Brand Infant Formula and upload pictures of
a receipt. Once the receipt is validated, they receive the value of the purchase,  up to $20 via Venmo. This is valid for one-time use per person every 12 months.
